POSITION DESCRIPTION
Conduct outreach by phone and mail for members who need to have a Health Risk Assessment (HRA) Completed. This person will help members who have received previous contacts attempts, to be reached and have an HRA completed.
ESSENTIAL DUTIES AND RESPONSIBILITIES
· Perform HRAs to SNP members to comply with SNP MOC 5 Stars Measures, Model of Care and established policies and procedures.
· Search for additional contact information using different resources that will be provided by the MSO.
Make the necessary efforts to obtain contact information of members to be used for the purpose of completing the HRA.
Resolve any issues that may be found during intervention with members or forward them to the appropriate department to be handled.
Work on their case load according to the Department Policies and Procedures.
Maintain appropriate records or logs of all intervention using the methods and forms provided.
Comply with Good documentation practices to demonstrate member’s outreach according to established procedures
Maintain good communication with all Departments.
Other task as required by the supervisor or manager.
EDUCATION AND EXPERIENCE
Associate degree (A.A.) or equivalent from two-year college or technical school; or six months to one-year related experience and/or training; or equivalent combination of education and experience.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Required to be fully bilingual, fluency in Spanish and English (read, write, speak) for Florida Business
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
INTERPERSONAL RELATIONS
Internal Contacts with Sales, Marketing, Customer Departments, Vendors and Providers.
GENERAL SKILLS
Strong interpersonal skills.
Excellent verbal and written communication skills.
Customer Service Skills: ability to practice, value and support service to both internal and external customers.
Persuasion Skills Time Management Skills: ability to establish priorities and accomplish tasks in a timely and accurate manner.
Team Work: ability to work in a team environment to create solutions to problems Computer Experience: Windows environment, Microsoft Word, Web-based applications.
PHYSICAL AND MENTAL DEMANDS
Requires sitting and standing associated with a normal office environment. Regularly talks and hears. This position requires that weight be lifted or force be exerted up to 25 pounds.
ENVIRONMENTAL AND WORKING CONDITIONS
Normal office environment Occasional evening or weekend work. The noise in the work environment is moderate.
